2025 MUSE Award Entry Form

Entrant Information

You must submit one form for each entry. This includes multiple entries to the same MUSE category or entries into different MUSE categories.
Please ensure the correct spelling of your name or desired pen name; any certificates or MUSE Medallions will include this spelling. The Cat Writers’ Association will not reprint certificates or awards for incorrect information.
Please note that the editor of an entry may not be listed as an entrant. A maximum of two individuals may be recognized with MUSE Medallions for an entry. For works with more than two creators, please list the lead names.
This should be a mailing address. This is the address to which any Certificate of Excellence or MUSE Medallion will be sent.
If you are not the person being considered for this award, enter your email here. Otherwise, we will contact the entrant if we need additional information or to notify that person of the entry’s status.
